Services

Support for Community Living Waiver & Michelle P. Waiver

The focus of SCL is to allow a person to remain in or return to the community. Persons with developmental or intellectual disabilities who meet the requirements for residence in an Intermediate Care Facility for persons with Intellectual Disabilities, and who meet other Medicaid requirements, are eligible for this program.


Supports for Community Living (SCL)/ Michelle P. Waiver (MPW) service can be delivered in the ways described below. Note: Certain services are exclusive to the type of delivery chosen below:


Participant-directed: This allows the individual to assist with the design of the programs, and to recruit, hire, train, manage, evaluate and terminate staff, and direct the delivery of services to meet the participant's needs. The services are specifically nonresidential and nonmedical.


Traditional:  Unlike with participant-directed services, the agency supporting the individual recruits, hires, trains, manages, evaluates and terminates staff.


Blended: This is a combination of different traditional and participant-directed services. For example, a person may choose to receive case management and physical therapy under traditional and choose to receive respite and personal care assistance under participant-directed services. 

Positive Behavior Supports

Amazing LIfe-KY, LLC utilizes a collaborative and evidence-based approach to support individuals who engage in challenging behaviors and/or lack the necessary communication skills to get their needs met. A Positive Behavior Support Specialist works with individuals to make positive changes in their lives and assists to ensure that all caregivers collaborate and provide consistency across all settings. Services include: a Functional Behavior Assessment, development of a Positive Behavior Support Plan, training for all caregivers, monitoring of plans. Collaboration may include caregivers within the home, school or work, and community.

Supported Employment

  • The goal of Supported Employment is to provide ongoing support services to eligible participants in supported employment jobs.

  • A Person Centered Employment Plan will be completed by a participant’s Supported Employment Specialist and updated as needed as required in the Supports for Community Living Policy Manual.

  • A Supported Employment Long-Term Support Plan will be completed by a participant’s Supported Employment Specialist and updated as needed as required in the Supports for Community Living Policy Manual.

  • A Supported Employment Specialist will document progress by submitting notes regarding a participant’s job development activity, participant’s job acquisition or stabilization activity, and a participant’s long-term employment plan.

Homemaker Services

Amazing Life-KY's homemaker service provides assistance in general household activities for clients who need such help to enhance their quality of life. These services are available to individuals enrolled in the Michelle P Waiver.

Respite Services

Respite services provide a variety of care services to help family caregivers that are physically and emotionally drained to relax and recharge. These services are available to individuals enrolled in the Michelle P Waiver and the SCL Waiver.

Community Living Supports

Community Living Supports (CLS) services are provided for children and adults through the Michelle P. Waiver and facilitate independence and promote community participation. This service is designed to allow individuals residing in their own home or their family’s home to have fun in life and learn skills that connect them to helpful resources. Services will include socialization, relationship building, and participation in community activities for a participant residing in his/her own residence or his/her family’s residence. Services will be therapeutic and provided in accordance to the person-centered plan. Community Living Supports will be offered at a one-to-one ratio and may include assistance, prompting, observing, or training in activities of daily living, such as bathing, eating, dressing, personal hygiene, shopping, money management, medication management, and transportation utilization. CLS services are provided by a Direct Support Professional (DSP).

Personal Assistance

Personal Assistance services enable a person to accomplish tasks that the person normally would do for himself or herself, if he or she did not have a disability. These include hands-on assistance; reminding, observing, guiding or training during an activity; assistance in managing medical care and transportation to access community services, activities and appointments, if these are not available under the Medicaid Program. Personal Assistance Services are available only to those who live in their own residence or in their family's residence.

Personal Care

Provides support and training working directly with individuals in their own homes to help them perform whatever tasks are necessary for successful implementation of their Individual Support Plans.
