Applying for Michelle P. or SCL Waiver
Note: Currently, there is a waiting list for MPW services and supports. Placement on the waiting list is determined by the date your completed application is received.
Step 1 - Obtain Financial Eligibility for Medicaid​
3 Options to Apply:

Apply online at Kynect (kynect.ky.gov) - You'll look for the type of Medicaid that pays for waiver services

Identification
Social Security Number
Income information
Resource Info - savings, checking, stocks, bonds, CDs, vehicles, land, etc.
Health insurance cards and premiums
Medical bills
Proof of Citizenship (if applicable)
Once Medicaid is received, proceed to Step 2
Step 2 - Apply for Michelle P. or SCL Waiver​
Option 1: Apply online at Kynect (kynect.ky.gov)
Look for purple Benefits icon and choose Apply for All Programs
Click Sign Up
You will create a Kentucky Online Gateway (KOG) account (if you don't already have one)
Click Apply for Benefits button
